If you are staying temporarily in the Netherlands, make sure that you have the European Health Insurance Card (EHIC). This card entitles you to the reimbursement of your costs for necesary medical care during your stay.
You are entitled to care that is covered by the Agis Basic Policy (comparable with the basic package established through the Dutch Healthcare Insurance Act
or the Dutch Algemene Wet Bijzondere Ziektekosten (AWBZExceptional Medical Expenses Act). Visit

For this the EHIC is not sufficient. In this case you need the E 112 form. With this form your health insurance fund will give approval for the planned health care. We advise contacting your healthcare supplier about this well before your departure to the Netherlands. It is important to note that the care must be covered by the Agis Basispolis or on account of the Algemene Wet Bijzondere Ziektekosten (AWBZ; Exceptional Medical Expenses Act) in relation to the E 112 form.
In some countries there is a separate insurance for accidents at work and occupational diseases. By issuing a E123 form there is a possibility to retain the claims of such an insurance in the Netherlands. On account of the E123 form the cost of medical care related to the accident are reimbursed. However, it is necessary that this care is covered by the Agis Basispolis or Algemene Wet Bijzondere Ziektekosten (AWBZ; Exceptional Medical Expenses Act).
The R 110 and R 123 forms apply to those who navigate on the Rhine. However, it is also possible that your insurance company will issue an EHIC (European Health Insurance Card) to you. As is the case with the EHIC, these forms entitle you to any necessary medical care that is covered by the Agis Basisplis or on account of the Algemene Wet Bijzondere Ziektekosten (AWBZ; Exceptional Medical Expenses Act).
The advantage of the EHIC is that you do not have to first pay the medical costs yourself. The care provider can claim from us directly. It is, however, important that you give the care provider a copy of your card within this context. The care provider requires this copy to claim from us. In some cases, the care provider may not be able to accept the EHIC. You can, in this case, send us the original bills with a copy of your card. For claiming costs, use ourĀ claim form.
